Student Information

A form must be filled out for each student attending. Do not list multiple names below.
If your first choice class is full, you will be placed inside of your 2nd choice class.
**Do not select same option as 1st Choice Class Selection.
Each student will receive one t-shirt in the size chosen above. Additional t-shirts are available for purchase below for $20 each.
If purchasing extra shirts, select the shirt size using the drop down menu. Additional t-shirts will be available for purchase at the academy for $20 each.
Application will not be processed until payment is received! If payment is made by check, please use this form as an invoice. After clicking the button below, a new window will open. A paypal account is not required. There is an option to pay with a credit card without signing in. Requests for a refund will be considered on a case by case basis. A full refund is not guaranteed.
